Patient-centered program for adults no longer in the acute phase of an illness but without the strength or independence to return home safely. Addresses each patient's physical, emotional and social health needs through specialized nursing care, physiotherapy, recreational therapy, occupational therapy and speech language therapy with the goal of returning home whether that be a private residence, retirement home or assisted living. |
