Description of Services: Supports for people living with type 1 and 2 diabetes, gestational diabetes, prediabetes or those at risk of developing diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian, and foot care nurse on staff * services offered virtually or by telephone
