Description of Services: Health First Chronic Disease Management is a unique program offering clients access to experience health professionals and physician specialty clinics to learn to better manage their lifestyle through diet, exercise, medications and stress.

Referrals can be made to any of the following programs:
  • cardiovascular rehabilitation
  • diabetes education and management - intake form
  • heart failure clinic
  • nutrition counselling
  • pulmonary rehabilitation


To register, fill out a referral form located on the website.

Eligibility - Population(s) Served: Anyone living with cardiovascular disease, diabetes, lung disease and heart failure
Application: Physician or NP referral for cardiovascular and respiratory services; Self-referral for diabetes services
