Program: Geriatric Assessment and Intervention Network (GAIN)

Description of Services: Provides an inter-professional approach to assessment and care for high-risk seniors, generally aged 65 and older who live at home or in a retirement residence; have multiple complex medical, functional and psychosocial problems; have recently experienced a functional or cognitive decline; or are at risk for falling, or have experienced falls.

Seniors will be assessed and treated as needed. Recommendations and treatment plans will be created and shared with the senior and primary care provider. The inter-professional team at the GAIN clinic includes:
  • Behavioural Support Nurse
  • Nurse Practitioners
  • Physiotherapist
  • Occupational Therapist
  • Social Worker
  • Personal Support workers
  • Pharmacist
  • Ontario Health atHome Care Coordinators
  • Geriatrician or physician who specializes in geriatrics available for consultation when required

Eligibility - Population(s) Served: Frail seniors living at home, 65 years or older, who are at risk due to multiple complex medical and psycho-social problems, unexplained decline in health or loss of capacity for living independently.
Application: Physician, nurse practitioner and self-referrals accepted
