Description of Services: Ontario Health atHome provides in-home and information and referral services by care coordinators at the Scarborough Health Network, Centenary Site.

The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home care services upon discharge from hospital and to link patients to other useful community resources.

In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language therapy, dietetic services and social work services.

Fees: Covered by OHIP
Eligibility - Population(s) Served: Must be a resident of Ontario with a valid Ontario Health Insurance Plan (OHIP) card. Must be a patient in the Northumberland Hills Hospital. Further eligibility criteria will be assessed upon initial intake according to the Long Term Care Act (1994).
Application: Although initial inquiry may come from patient/family member, a formal referral may be required from a hospital professional such as the physician, nurse, or social worker.
Waiting Period - Case management services are available prior to hospital discharge
