Description of Services: Consultation by a specialized geriatric nurse within a hospital emergency room to provide assessment, diagnosis, identification of "frail and at risk" elderly, and to initiate appropriate treatment in the hospital or community

Eligibility - Population(s) Served: Patient has to be 75 yrs or older, and have one or more of the following: live alone, difficulty walking, functional decline, frequent falls, 5 or more medications, history of cognitive impairment, suspected elder abuse or neglect, ED use in previous 30 days, as well as an RN or any other emergency's team member professional recommendation
