Organization: Brock Community Health Centre
Description of Services: Specialized geriatric services to the frail elderly residents in the Township of Brock and surrounding area. Comprehensive geriatric assessment in a clinic environment with a specialized nurse practitioner. Access to a geriatrician via teleconferencing (OTN) in a clinic setting. Home assessments by an interdisciplinary team, follow-up and support, links with the client's current primary care provider to other specialized geriatric services.

Eligibility - Population(s) Served: Frail, elderly individuals, primarily aged 65 years and older, who live at home or in a retirement residence, have multiple medical problems, have experienced a recent decline in their independence or a recent fall, and have limited social supports.
Application: Physician referral, self-referral and referrals from family members.
