Description of Services: Ontario Health atHome provides in-home, information and referral, and placement services by care coordinators in Haliburton Hospital.

The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services, and/or placement services, upon discharge from the hospital and to link patients to other useful community resources.

In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language pathology, nutrition and social work.

Fees: Covered by OHIP
Eligibility - Population(s) Served: Must be a resident of Ontario with a valid Ontario Health Insurance (OHIP) card. Must be a patient of the Haliburton Hospital. Further eligibility criteria will be assessed by the case manager during the intake process based on the full eligibility criter
Application: Although initial inquiry may come from patient/family member, a referral may be required from a hospital professional such as the physician, nurse, or social worker.
Waiting Period - Case management services are available prior to hospital discharge.
