Description of Services: |
Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services * They also assist the client in planning for discharge when services are no longer required * Care coordinators are responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services * The services provided through Ontario Health atHome may include:

Eligibility - Population(s) Served: | Visiting services are available to residents of Ontario of any age who have a valid Ontario Health Insurance (OHIP) card. Further eligibility criteria will be assessed by a case manager during the intake process based on the full eligibility criteria | ||

Physician, hospital or self-referral by phone, fax or e-mail. Waiting Period - No waiting time to make a new referral. Waiting periods for direct service vary depending on type of service required and priority need. |
