Description of Services: |
Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services. They also assist the client in planning for discharge when services are no longer required. Care coordinators are responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services. The services provided through Ontario Health atHome may include:

Eligibility - Population(s) Served: | Visiting services are available to residents of Ontario of any age who have a valid Ontario Health Insurance (OHIP) card. Further eligibility criteria will be assessed by the care coordinator during the intake process based on the full eligibility criteria a | ||

Physician, hospital or self-referral by phone, fax, e-mail. All written and phone referrals should be directed to Central Intake. In person referrals may be directed to the Haliburton Office.
Waiting Period - No waiting period to make a new referral. |
