Description of Services: The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services, and/or placement services, upon discharge from the hospital and to link patients to other useful community resources. In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language pathology, nutrition and social work.

Eligibility - Population(s) Served: Visiting services are available to residents of Ontario of any age who have a valid Ontario Health Insurance (OHIP) card. Further eligibility criteria will be assessed by the case manager during the intake process based on the full eligibility criteria a
Application: Although initial inquiry may come from patient/family member, a formal referral may be required from a hospital professional such as the physician, nurse, or social worker.
