Description of Services: |
Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services. They also assist the client in planning for discharge when services are no longer required. Care coordinators are also responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services. The services provided through Ontario Health atHome may include:
