Description of Services: Planning and support for young adults with a developmental disability between the ages of 18-30 to ensure successful transition from school to community life * Participants gain knowledge of how to access community services, participate in the community, and work on building confidence and self respect * community activities include utilizing community services that include transportation, shopping, educational and recreation facilities, doing volunteer work, researching community employment programs, and learning street safety and awareness.

Eligibility - Population(s) Served: Must have an intellectual disability and be between 18 - 30 years of age.
Application: Clients or family can contact office.
Waiting Period - 6 - 12 months.
